The Elmhurst University Sports Medicine staff is responsible for the prevention, evaluation, treatment, and rehabilitation of injury and illnesses of our intercollegiate student-athletes. We strive to provide high quality medical care for the physical, mental, and emotional well-being of our student-athletes.

The Elmhurst University Sports Medicine staff consists of a team physician and four full-time athletic trainers. In addition to medical care, the staff provides medical coverage of practices and competitions.The athletic trainers are located in the main athletic training facility on the first floor of R.A. Faganel Hall.

Medical Forms - Health Information

Elmhurst University student-athletes are required to complete online medical forms through Hub - Compliance and Recruiting. Student-athletes must obtain a pre-participation examination from an MD, DO, PA and FNP. Only a physical from these healthcare providers will be accepted. Please provide the attached physical during your examination. This is the ONLY physical form that will accepted in order to participate in athletics at Elmhurst University.

Health Insurance Policy

Elmhurst University mandates that all full-time students have health insurance. It is the responsibility of the student-athlete and their parents/guardians to provide primary insurance coverage. The Athletic Department provides secondary insurance coverage for ONLY injuries that are a direct result of participation in intercollegiate athletics. This policy is secured through First Agency, Inc., of Kalamazoo, Michigan. Kayla Vallines works with student-athletes and families to process paperwork and file insurance claims. The policy through First Agency, Inc, will only take effect after all claims have been acted upon by the primary insurance company, and the appropriate information has been given to the athletic training staff. It is the student-athlete and their parents/guardians responsibility to see that all bills are submitted to their insurance company. Failure to do so will relieve the Athletic Department of liability to pay benefits.

The Athletic Departments liability for medical expenses resulting from injuries sustained by athletes is defined in the following manner. Liability is extended to cover:
  1. Medical expenses from athletically related injuries only when the student is enrolled in school at Elmhurst University.
  2. Only those injures reported to the athletic trainer or team physician within forty eight (48) hours from the injury producing incident if, and only if, it is a direct result of intercollegiate athletics participation.
  3. Those injuries received during the athletes season which in the opinion of the team physician and the Athletic Director are directly attributable to participation in intercollegiate athletics while a full time student at Elmhurst University.
  4. Medical expenses resulting from services which have been authorized by the team physician through a referral to a medical specialist.
  5. Payment of medical expenses not covered by primary insurance, but authorized by the team physician and full time athletic trainers. (Payment may be partial or full, depending on the nature of the injury and is determined by First Agency, Inc.)

Sickle Cell Trait Information

According to the National Collegiate Athletic Association (NCAA) rules, all Division III student-athletes are required to be tested for sickle cell trait through a blood test. NCAA Sickle Cell Fact Sheet.

ADD/ADHD Medical Exemption

In accordance with an NCAA mandate, all Division III student-athletes must provide specific documentation of any prescribed medication for the diagnosis of ADD/ADHD before they participate in any intercollegiate athletics event, including strength and conditioning sessions, practices, and competitions.
